Contact and license details, three-year residence history, experience, accident and violation history, employment history, and applicant signature. Add employment pages as needed to cover all employers for three years and earlier CMV employers back ten years.

Fillable history authorization ↓Complete and sign the fillable questionnaire. Merci arranges required testing and the Clearinghouse full query; your electronic query consent is given in the official Clearinghouse.
Fillable questionnaire ↓Merci must verify the required state driving records and CDL medical status. Applicant copies do not replace employer verification.
Full checklist ↓Complete the prior-seven-days hours statement when instructed. Recruiting also coordinates qualification checks, road-test requirements or permitted equivalents, and company onboarding.
Fillable prior-hours form ↓Complete these during onboarding when instructed. For Form I-9, you choose acceptable documents from the official lists. Direct deposit, emergency contacts, and company policy acknowledgments are provided separately by Merci.
